Harry R. Jacobson, M.D.
Dr. Jacobson is the founding Vice-Chairman of CCA. He co-founded and was Chairman of the Board of
Directors of Renal Care Group, Inc. from 1995 to 1997, and he continues to be a Director of Renal Care
Group. He serves as Vice Chancellor for Health Affairs at Vanderbilt University, a position he has held
since 1997. He also currently serves as professor of medicine at Vanderbilt University Medical Center,
a position he has held since 1985. Dr. Jacobson received a Bachelor of Science degree from the University
of Illinois and his M.D. from the University of Illinois Abraham Lincoln School of Medicine. He completed
his internal medicine training at Johns Hopkins Hospital and his nephrology training at Southwestern
Medical School in Dallas, Texas.

Sam A. Brooks (1939-2003)
Mr. Brooks was Renal Care Group's chief executive officer beginning in 1995 and assumed
the position of chairman in 1997. Prior to forming Renal Care Group, Mr. Brooks established and
was president of MedCare Investments, Inc., a health care investment company, from 1991 to 1995.
From 1969 to 1986, Mr. Brooks was with Hospital Corporation of America (HCA), ending his tenure
there as vice president and chief financial officer. He began his career in 1962 in the Dallas
office of Ernst & Ernst with principal responsibility for healthcare clients.

John H. Dayani, Sr., Ph.D.
Prior to co-founding CCA, Dr. Dayani was the founder and chief executive officer of Total eMed,
Inc., an Internet-based medical transcription service funded by Electra Fleming Capital Health
Partners and later sold to MedicaLogic/Medscape (MDLI – NASDAQ) in May 2000. Prior to
founding Total eMed, he was the founder and chief executive officer of MEDIFAX, Inc., inpatient
medical record services funded by Welsh, Carson, Anderson and Stowe sold to MedQuist, Inc.
(MEDQ – NASDAQ) in 1998. Dr. Dayani co-founded a homecare and nursing staffing company
in 1983, which he sold in 1990. In addition to founding three healthcare companies, Dr. Dayani
was the acting president of Kimberly Nursing prior to its sale to Lifetime Corporation in 1981.
Dr. Dayani earned his B.A. and Ph.D. in engineering from Vanderbilt University. |

David J. Maron, M.D.
Dr. Maron received his undergraduate degree from Stanford University and his M.D. from the Keck
School of Medicine of the University of Southern California. He completed his internal medicine
training at UCLA. He was a Robert Wood Johnson Clinical Scholar at Stanford University, where he
also completed his cardiology training. Dr. Maron joined the faculty in the Department of Medicine
at Vanderbilt University Medical Center in 1993, where he was Medical Director of the Kim Dayani
Center for Health Promotion and promoted to Associate Professor of Medicine. He is currently on
leave of absence from Vanderbilt. |

Douglas L. Koppang, Jr.
Mr. Koppang is an executive with over twenty years of healthcare experience. He has
extensive experience in public accounting, and he has been the Chief Financial Officer of
several healthcare companies based in Nashville, TN. Mr. Koppang received his B.A. degree in
economics and business administration from Furman University, and his J.D. degree in law from
the University of South Carolina. |

Inland Cardiology Associates was established in 1981 and has grown to 15
cardiologists in 4 offices and 15 visiting clinics serving eastern Washington, northern Idaho and

Cardiovascular Institute of the South, Houma, LA
Cardiovascular Institute of the South is one of the largest, most comprehensive and most advanced centers for the nonsurgical and surgical treatment of heart and vascular disease in the US. Cardiovascular Institute of the South was founded in 1983 by Dr. Craig M.Walker in the small southern Louisiana city of Houma, an area which has one of the highest incidences of cardiovascular disease in the industrialized world. That practice, Houma Heart Clinic (now Cardiovascular Institute of the South/Houma), grew within a single decade into a group of more than 30 cardiologists and cardiovascular surgeons representing every specialty in heart and circulatory medicine, with clinics in more than a dozen communities.
From the outset, Dr. Walker envisioned the Institute as a center for the development of new and improved techniques and technologies for the treatment of heart and peripheral vascular disease. The Stern Cardiovascular Center, Memphis, TN
Originally known as The Cardiology Group of Memphis, the practice was renamed The Stern Cardiovascular Center to commemorate the legacy of excellence in cardiovascular medicine bestowed by its founder, Neuton S. Stern, M.D. (1890-1969) and his son, Thomas N. Stern, M.D.
Neuton Stern was a pioneer in modern cardiology. In 1919 he demonstrated the city's first ECG machine. He was Tennessee's first diplomate of the National Board of Medical Examiners and was a founding member of the American Heart Association.
The Center provides state-of-the-art diagnostics and treatment of all aspects of cardiovascular disease, including outpatient cardiac catheterization. |

Thomas W. Lyles, Jr.
President, MedCare Investment Funds
Mr. Lyles is President of MedCare Investment Funds and President and
General Counsel of Mission City Management, Inc. in San Antonio, Texas. Mr.
Lyles has managed an investment portfolio for over 10 years, including both
public and large private companies. Mr. Lyles earned an accounting degree
from Texas A&M University, where he graduated Summa Cum Laude. He received
his Juris Doctor, Cum Laude from S.M.U. School of Law, and subsequently was
appointed Briefing Attorney for the Chief Justice of the Supreme Court of Texas.
In addition, Mr. Lyles is a Certified Public Accountant and has written material
for the Texas State Bar and has been a speaker for the State Bar of Texas and
the Dallas Bar. Mr. Lyles serves on numerous for-profit and non-profit Boards
of Directors. |

Craig M. Walker, M.D.
Medical Director, Cardiovascular Institute of the South
Dr. Walker, is a graduate of Nicholls State University and LSU Medical
School. He trained in cardiology at Ochsner Foundation Hospital and completed
a research and teaching fellowship at Harvard Medical School. He founded
Cardiovascular Institute of the South in Houma, Louisiana. Dr. Walker was
named one of the Ten Outstanding Young Americans for 1992 by the U.S.
Junior Chamber of Commerce. He serves on the medical advisory boards of
several medical device companies. He is a Fellow of the American College
of Cardiology, the American College of Physicians, the American Society
of Cardiovascular Interventionists, the International College of Angiology,
the Society for Cardiac Angiography and Interventions, the American
College of Chest Physicians, the American Society for Laser Medicine and
Surgery, and the American Heart Association Council on Clinical Cardiology.
He is a world renowned expert in peripheral vascular interventions. |

Michelle M. Molden
Michelle M. Molden has served as president and chief executive of Piedmont
Heart Institute of Atlanta, Georga since August of 2007. Ms. Molden joined
Piedmont Healthcare in December 2002 and served as Executive Vice President
and Chief Administrative Officer. Ms. Molden earned her undergraduate degree
in Microbiology at Miami Universityh in Cincinnati, Ohio. She earned her MBA
from Kennesaw University in Kennesaw, Georgia. Piedmont Heart Institute is a
joint-venture partner with CCA in CCA of Atlanta, LLC, which operates an
out-patient diagnostic cath lab.

Steven S. Gubin, M.D.
Steve S. Gubin is a cardiologist with the Stern Cardiovascular Center (Stern),
focusing on preventative cardiology, promoting proactive patient education to
reduce risk factors. He received his medical degree from University of Tennessee
and undergraduate degree from UNC Chapel Hill and has been practicing medicine
since 1985. Dr. Gubin received the Program Directors Award for Outstanding
Contribution to the Residency Program at St. Joseph Mercy Hospital, Ann Arbor,
Michigan in 1988. CCA manages the outpatient cardiac cath business of Stern.

Bruce Edward Murphy, M.D., Ph.D., F.A.C.C.
Born in El Dorado, Arkansas, Dr. Murphy graduated with honors from the University of
Arkansas for Medical Sciences in Little Rock. He is Board Certified in Internal Medicine,
Cardiovascular Disease, and Interentional Cardiology. In addition to his Medical Degree,
Dr. Murphy earned a Ph.D. in Pharmacology. At UAMS, he completeed an Internship and
Residency in Internal Medicine as well as a Fellowship in Cardiology. In addition to
his private practice at Little Rock Cardiology Clinic, he is the Medical Director at
Arkansas Heart Hospital, President of Little Rock Cardiology Clinic, and Assistant Professor
at UAMS. Dr. Murphy has had intensive interventional training experience with world-renowned
cardiologists and has athored and contributed work to several medical publications. He trains
cardiologists regularly from all across the United States and the world on advanced interventional
techniques.

Steve Blades
Executive Vice President and Chief Operating Officer
Steve has an extensive background in healthcare operations, organizational governance
and culture, compliance, and national/state regulatory affairs. He has worked in
healthcare administration for over thirty-five years, including more than a decade
as part of the United States DHS, Health Care Financing Administration and the Office
of Inspector General. He has served as the administrator of a large academic family
medicine program in North Carolina and as chief executive of two large cardiology
practices in Tennessee and Texas. Steve was a co-founder and past president of the
Cardiology Leadership Alliance (CLA), an organization representing major cardiology
practices and is currently president of the Cardiovascular Outpatient Center Alliance
(COCA), an organization representing non-hospital outpatient cardiac catheterization
facilities throughout the country. Steve earned a BA in Government from Florida
State University and a MPA from the University of Georgia.
